Passport, ID & Emergency Contact Form (Required)
Passports are required for International Travel for all guests including children of any age. Passports must be valid for at least6 months after your cruise departure date. U.S. Citizens may obtain more information on obtaining a U. S. passport, by visiting the U.S. Department of State website.
Due to increased security measures the U.S. Coast Guard requires that each person traveling with Cruise West complete a Passport/Identification & Emergency Contact Form at least 45 days prior to your departure date. This information is mandatory for all guests who travel with Cruise West. Please take this request seriously and complete the form as soon as possible. If the information is not received, the Coast Guard reserves the right to deny passengers to board the vessel. Completing this form takes only a few minutes and can be done by clicking on the link below, supplying the requested information and submitting the form online. That's it.

Booking Your Flight Refundable, changeable air arrangements are available when you book your flights through Cruise West. Unlike most air tickets, our air tickets are changeable and even refundable in most cases. You'll be assured smooth connections throughout your trip, you can decide to extend your stay, either pre- or post-tour, or interrupt your homeward journey with a few days' stopover. More peace of mind to make your vacation as stress-free as possible.
If you have any travel plans that were NOT booked by Cruise West, including flights, rail, you are driving to the port, staying with your friends, etc., please submit a Flight Information Form.

Special Physical Needs The Special Physical Needs Form helps identify any special accommodations needed on a Cruise West cruise or tour as the result of a disability or medical condition. This information will assist us in providing the best service and the safest environment possible. Our primary concern is the health, safety and well-being of our guests and crew. Cruise West does not discriminate against persons on the basis of disability. Due to limited medical facilities onboard our vessels, guests with serious medical conditions or women with pregnancy complications or in the third trimester of pregnancy should consult their physician to determine whether travel is appropriate. Guests are also advised that personal services and devices, such as wheelchairs, cannot be provided by Cruise West.
Guests in need of services of a personal nature, such as assistance in eating, toileting, or dressing, are responsible for arranging for such services and are encouraged to travel with a companion for such purposes.

Special Dietary Needs Our chefs will do their very best to address your dietary needs and concerns. Please keep in mind that due to many of the areas we cruise in, shore side hotels, and land tours may have limited food preparation facilities, and perhaps limited special product availability. We may have to adjust or alter your request, but will make every effort to meet your dietary needs. Please ask to see the Chef at the time you board the vessel. Special dietary needs we can accommodate include bland, non-spicy, diabetic, low cholesterol, low fat, sodium free, low calorie, lactose free, gluten free, vegetarian, food allergies, and some alternative vegetarian. Submit a Special Meals Form to alert the Chef to your special dietary needs.
